An outsourced kidney education partner

Extend your kidney education program without adding another full-time educator.

What NephroReach is

An outsourced kidney education partner, not just software.

Through NephroReach, healthcare organizations extend their patient education capabilities while giving patients structured learning, ongoing education resources, and supportive digital tools.

Educator-led education

Structured kidney and dialysis education led by real educators — built for the organizations and the people who need it, starting with nephrology practices.

Direct communication

Messaging between patients and the organizations that care for them, so information about appointments, travel, and questions does not depend on phone tag.

Self-management tools

A platform in development for patients to track blood pressure, weight and fluid, medications, dialysis-related tasks, and education progress alongside the curriculum. Not yet available.

“NephroReach is the platform. Education is the service.
Why organizations partner with us

Who we serve

Four ways healthcare organizations, patients, and caregivers work with NephroReach.

Every program below is in development or under review. None are open for enrollment yet — reach out and we'll follow up as each one opens.

Under review

Nephrology Practices

An extension of your practice's kidney education resources, not a course license. NephroReach provides the structured CKD and dialysis-preparation education service, built around the 21 Day Dialysis Journey, with patient participation and progress reporting planned for participating practices. Practices also have the option to add the Dialysis Crash Course for urgent or transferred patients. Currently under review before launch.

Priced per organization, billed monthly, once available. See the curriculum ↓

Dialysis Facilities

An extension of patient engagement and communication between dialysis treatments. In development to support patient–facility communication, education, dialysis-related tracking, and workflows such as dialysis travel request coordination.

In development. Tell us about your facility and we'll follow up as this opens.

Patients & Caregivers

Continued kidney education, self-management support, and caregiver participation, with individual access options in development. Not yet open — joining the interest list does not enroll you in anything.

We'll reach out as individual access becomes available.

Hospitals

The Dialysis Crash Course, built for patients and families facing an unplanned dialysis start during a hospital stay. Patients who complete it continue automatically into the 21 Day Dialysis Journey as they transition to ongoing nephrology care — one education service, not two separate handoffs.

In development. Tell us about your hospital and we'll follow up as this opens.

Core education curriculum

Built around the 21 Day Journey, with the Crash Course available too.

Once open, your team will enroll planned transitions into the Journey. Practices also have the option to add the Dialysis Crash Course for urgent or transferred patients.

Under review · 21 day pathway

Dialysis Journey

The core of the nephrology-practice service. For planned transitions: three weeks covering modality choice, access planning, transplant referral, working and traveling on dialysis, and preparing the home.

Built for the CKD stage 4 and 5 population your practice already follows.

Under review · 5 day pathway · optional add-on

Dialysis Crash Course

Built primarily for hospitals and unplanned starts. Practices also have the option to add it for urgent or transferred patients. Five short daily lessons: why dialysis became necessary, how hemodialysis works, protecting the access, fluid and diet basics, and what to call about.

Patients who complete it continue automatically into the Dialysis Journey.

How the practice program will work

Four steps, once it's open, and only the first one is your practice's to do.

1

Your team enrolls the patient

A coordinator selects the pathway and enters the patient from the organization dashboard. It takes under a minute.

2

The patient works through it at home

Short lessons arrive one at a time, on the phone the patient already carries.

3

You see who finished and who stalled

Progress is visible per patient, so the follow up call goes to the person who needs it instead of everyone.

4

Clinic conversations start further along

Patients arrive already knowing what an access is and why the fluid limit exists, so staff stop repeating the basics.

Coming to the platform

Live education sessions, and tools patients use between lessons.

Two pieces of NephroReach we're building out now, both grounded in the same curriculum. Once they're live, this is also where the Access NephroReach button above will take you.

Coming soon

Educator-led sessions

Structured CKD and dialysis-preparation education, plus recurring educator-led sessions — renal nutrition, caregiver education, transplant education, dialysis Q&A, Thriving on Dialysis, curriculum recap, and expert-panel Q&A among them, with optional scheduled live sessions led by real educators.

Self-management tools for patients

A companion app to the curriculum, with a secure, individual login for each patient, for tracking blood pressure, weight and fluid, medications, and dialysis-related tasks, plus education progress and caregiver resources — built to sit alongside a patient's care team, not replace it.

How pricing works

Priced per organization, billed monthly.

Every plan is a monthly subscription per organization, with no per-patient fee, whether that's a nephrology practice, a dialysis facility, or a hospital. Every package below is available to request information on now — individual patient pricing is still in development, and we'll follow up as it opens.
